Advance Care Planning is the process of thinking about what matters
most to you, identifying who would make healthcare decisions if you could not, and having conversations that help them understand your values and wishes.
You do not need to be older, seriously ill, or approaching the end of life to begin. Accidents, sudden illness, and unexpected medical emergencies can happen at any age. Planning now helps the people you trust make future decisions with greater confidence.
Planning Is For Every Adult
Many people associate Advance Care Planning with aging or terminal illness. In reality, it is an ongoing process that can begin at any stage of adulthood and be revisted as your health, relationships, and priorities change.
Planning ahead may be especially helpful if you:

You do not have to complete the planner alone. There are reliable Advance Care Planning tools designed to help Ontarians reflect on their wishes and prepare for conversations with their Substitute Decision-Maker.
During our session, we can work through an appropriate planner together. I will help you understand the questions, explore what matters most, and identify areas you may wish to discuss further with your family, lawyer, or healthcare provider.
Ontario resources describe ACP as including two central steps: identifying or confirming your future Substitute Decision-Maker and helping that person understand yoru wishes, values, and beliefs.

Frequently Asked Question
Do I need to be sick to complete Advance Care Planning?
No. Advance Care Planning is most useful when it begins before a crisis. Starting while you are well gives you time to reflect and ask questions, speak with the people you trust, and make your wishes known without the pressure of an urgent medical situation.
